Healthgram (FKA: Primary PhysicianCare - PPO)
SEQC entered into a group agreement with Primary PhysicianCare on June 1, 2003. Effective February 1, 2016, fees are based on a percentage of the current North Carolina Medicare fee schedule for office based services. This is a substantial improvement over the initial fee schedule which was based on 1999 NC Medicare. Each physician affiliate must execute a Provider Services Adoption Agreement indicating his/her agreement with the terms of the contract as executed by SEQC.| Physician | Specialty | Practice |
